West Midlands Police inspector guilty of misconduct after escaped Birmingham patient killed by train

Inspector Collin Baker from West Midlands Police has been found guilty of misconduct in relation to decision-making failures which meant Matthew Caseby was not found before being killed by a train near University Station.
